Favorite fantasy cover artist right now is Marc Simonetti:

Another favorite and one of the first artists I ever got art from is Ciruelo. I love the covers he did for Michael Stackpole's Dragon Crown War books, mainly this one:

I haven't seen much album art in a long time, and none really stick out in my head.

 

I love just about every book with cover art done by Elmore. But some of my favorite book art was done by Keith Parkinson.

I absolutely  loved the original Elmore art for the Dragonlance Chronicles. In fact his art is probably what drew me to the series. In general I will always have an incredibly powerful love for anything Mr. Elmore does. When I donated all my AD&D stuff to the troops I only kept a couple of Dragon Magazine issues that had particularly nostalgia inducing (the seasonal ones in particular). And the Keith Parkinson covers for the Eddings books. 

 

I also absolutely love the Michael Whelan covers for Memory, Sorrow, and Thorn. Particularly the cover for Stone of Farewell. 

 

There is soooooo much art that I love I could easily spend all day listing it, but these are some of them.

Since everyone is mentioning Tanith Lee, there is one particularly fantastic cover art for the book Dark Castle, White Horse, consisting of two stories (The Castle of Dark and Prince on a White Horse). One story is dark and foreboding, the other is highly whimsical. Surprisingly, I had found it in my sister's old room when I had taken it over, I never thought her as one who ever read fantasy. It was an amazing read when I was younger, and the stories tend to draw you in. I'll let the cover art (created by Ken Kelly) spark your imagination:

I'd probably have to go with Thousand Foot Krutch's "Set It Off" album cover... The one from 2001, not the re-re-release in 2004.  There are others that are nice, but that one's probably my favourite.

 

....And not because it's all graffitied inside the cover :P

Love the Elmore covers, (especially the "Elminster in Hell" one,) but I find Weis and Hickman dreadfully dull to read.

I have to start with a couple of classics

 

Holy Diver

Probably my favorite though is Follow the Leader from Korn (I always found this eerie and haunting for some reason)

I would also say anyone looking for cool fantasy/sci fi art should check out the Spectrum fantasy art books.  There is some really beautiful artwork in them (they do contain some adult content though)

Way too many to narrow it down easily, but here are a couple examples:

As mentioned, Iron Maiden has always had some awesome album covers. This one is a particular favorite; there are a lot of little Easter eggs in that painting for fans to discover.

There's like, way more than this, but I can't think past this throbbing headache, so these two will have to do. 

 

Book: A Fisherman of the Inland Sea, by Ursula K. Le Guin.

I always thought the art for this was magical. 

 

 

Album: The Memory of Trees, by Enya.

This cover is a riff off a Maxfield Parish painting, the name of which I do not recall right now. The album title, combined with the art, made me imagine some kind of magical sect where they recorded the memories of sentient, talking trees.

Best I could find online.  Thankfully it's on Wikipedia.  I would have tried to find it, but sadly it would seem it's in the storage container that's in the back of the closet, and as much as I want to be able to show it off (as well as the "graffitied" insides (aka signed inside)) I'm actually listening to the advice of "take it easy for a few days" so that storage bin will have to stay where it is for now.  I have my 2004 cover album readily available as apparently a while back I was pulling FLAC files of it for my audio library, but like their albums after Phenomenon I'm not really a fan of it.  Okay, Art of Breaking had a neat-ish cover, but still.

 

Glad someone else posted Rust in Peace, that one's pretty awesome ^_^
